Addition rejected and also added

Several times now, I've tried to add items (ie keywords) to a Title.  The additions are rejected due to the "Title already has" it.  Then I go look at the Title and yes, it does already have it.  So the reviewer sees my change request, performs the change him/herself, and then rejects my change request as redundant?  Is that just how it works?  I'm pretty positive these attempted additions were not already present.
