Approved photos not gone live.

I submitted three photos for a Gunsmoke episode S11.E9 and they were approved but never gone live.

I submitted them twice, both times they show "Approved for Publication", but never gone live.

I seek three types of help.

First, please IMDb staff, fix this anomaly. 

Second, why does this happen, what is the cause of this anomaly? (I cannot make a career of adding photos twice and following-up for weeks.)

Third, when I examine the Submission Record, for each of the six there are mostly "Approved" entries but always ONE Declined entry that reads like the text copied-in below.  What is the meaning of that "declined"?  Am I submitting photos in some errant way?  I was never trying to make a "Title Link Addition" -- or even know how one would!
