Cast Member Needs To Be Removed From Listing

Someone other than the producer add someone to cast member on a project I am working on the the role is incorrect for the cast member added. It has been submitted twice now to have it removed to address the issue and it still has not been removed. How can I get it removed since submitting it for removal has not yielded having it removed. Both requests say approved in status yet it still remains.
