Cast member's name being repeatedly declined

Hello! One member of our title's cast is being repeatedly declined without a reason given. My only guess is a warning on the submission form that says "Rev" is usually considered an honorific title and not allowed, but we have uploaded evidence in the form of on-screen credits that Rev is a first name, not a title.
