Credit incorrectly included in my IMDb

Question

There was a credit incorrectly added to my IMDb. I did not participate in any capacity in the production. My name and picture also appear in the Title page which I would also like to delete. I submitted a request to delete these credits in my page (#250815-235230-649000), but was denied saying the information was unable to be verified. Given I did not work in the production, there's no evidence I have in support thereof. Thank you for the help.
