Crew member addition declined, despite credits evidence

Hello there, got another crew member addition problem. For House of Mouse, one of the show's storyboard artists was missing in the database, being Carlos Baeza, so I added all of his episode credits. Yet again, they declined because "unable to verify", even though I have used the show's credits as a source (shown below here). This needs to be fixed. Please help.
