Follow up post to add new episodes

Hello, I recently tried adding this week's new soap opera episodes. I have tried submitting this week's episodes but got denied for "bad formatting." The titles aren't even formatted, they are just the date of the episode and sometimes just the episode number 1.xxxxx. What exactly do I need to type to consistently get approval to add soap opera episodes?
