Listing a birth date may not be essential for work contacts, but there is an upload link in the form where you could include a photo of an ID, for example.

You could include the same link for a name correction, which may work. If the name is spelled correctly in the screen credits, an image of the credit would be the best evidence since IMDb is mainly concerned with how the name is written on screen.

Photos can only be submitted to a name page with a paid IMDbPro subscription.
