Image Both Approved and Declined!

Ironically with this post being about an image issue, the option to add an image to these posts seems to have vanished! So, I'll have to try and describe it. This image #250515-224103-074002 says Tick - Approved for Publication Then below it says Declined - Reason Does not meet contribution guidelines So which is it?
