My title was approved and then declined

I had submitted #260609-175408-321000 and it was approved but then it never showed up. This was in early June, so I figured I'd give it enough time to process. I've never had any issues submitting in the past, so I'm wondering what happened to my submission. I posted a link to view the film on Vimeo, but if I need to provide more information on the short film I can. I just am not sure what happened during the processing. Thanks.
