Unsuccessful submitting my pilot episode as a writer/Producer

Hi, I have failed to fill out the form for two questions, Following instructions and yet it does not want to accept the ORDER# - us(USA). Failed repeated attempts to answer the question different ways, adding different attributes, but it just does not want to let me through. I have pretty much the whole form resolved (Green color).
