Help! I can’t remember this movie or tv episode..

I think it was made in the 80’s or 90’s. I only remember a couple of scenes. They center around a kid that sits upside down in chairs. I think it’s a little girl but it could be a boy. I think they had very short, brown hair. I think this kid is neglected or lonely or has some health issue. One scene takes place at a hospital. The family is visiting someone and this kid is too young to go back so they wait alone in the waiting room. I think a nurse or Dr. prescribes them ice cream.
