Movie about old man reading a book to old lady

So here is what i remember:

There was this older guy {i think he wore glasses} that had the job to come to this old womans house to read her books. He is reading the book and the whole book sounds familiar to the old lady.
The book is about a guy and a girl falling in love. At the end the woman realizes that everything that happened in the book did actually happen to her {she is the girl from the book}, and that the man that was reading the book to her is actually a guy from the book himself! He, the old man ,is her past lover!!

Im sorry if this all sounds confusing and everything english is not my main language. But i would be grateful if someone could help me since i am looking to find this movie for about 10 years
