How to do it while the image is uploading:

To upload and set your primary image while logged in to your membership, please go to your IMDbPro page and select the “Images” tab.

To add an image, click on the gray icon box that says "Add Image".

After selecting your image, check the box under "About this Image" that reads "Set as primary image..." for your name page.

How to do it if the image is already uploaded: 

To set a previously uploaded image as your primary, while logged in to your membership, please go to your IMDbPro page.

Select an image to open the image viewer and select "Set as primary" for the image you're viewing.

You can also access the Primary and Featured Images tool by selecting the "Edit" icon above your "Featured Images" section on your IMDb name page.
